What we ask of you
  • Valid Minnesota cosmetology, esthetics, or applicable professional license
  • Professional liability insurance — min. $1M per occurrence
  • LABB Collective listed as additional insured on your policy
  • Signed LABB license agreement (not a traditional lease)
  • Business entity info or SSN for payment and 1099 purposes
  • Emergency contact on file with LABB
Injectable Suite (Room 1)
Additional for injectors
  • Copy of NP, PA, RN, MD, or DO license — current and in good standing
  • Medical malpractice insurance — min. $1M / $3M limits
  • Supervising physician agreement if required by your scope
  • Sharps disposal and medical waste plan — your responsibility
  • HIPAA compliance acknowledgment for protected health information
  • DEA registration or documentation of non-controlled substances only
